Please also include your signed distribution agreement and then select your category payment after you submit the form: Musician-Author-Filmmaker Distribution Agreement 

Instructions:

• Click the above agreement that takes you to a new landing page.

• Then move your mouse cursor to the top right corner and select the download button once it appears.

• Then download the pdf file to a new folder location on your computer and open it. (Do Not Fill Out the Form on Your Browser)

• Now fill out agreement form on your new folder.

• Then select the blue submit button on the form.

•  Finally select your category for payment: Musician ~ Author ~ Filmmaker on the form that will that you to a new T25CL landing page to make a payment.
